A number of my patients seek treatment for OCD for the first time when they are faced with making a major life decision, such as getting married. They are consumed with doubt and anxiety about making “the right choice.” Since OCD typically waxes and wanes over one’s life, the content of the obsessions usually changes, and symptoms can range from mild to severe, these folks may not be aware they have OCD. Important transitions in one’s life can set OCD into motion. The decision to get married is one of life’s major transitions and often OCD will manifest itself around needing certainty about the relationship.

Regarding the decision to get married, OCD demands that there be no doubt in a person’s mind whether he/she has chosen the right person to marry. To the OCD sufferer, their obsession is, “I must know for sure that my fiancée is the perfect person for me to marry.” Because no person is perfect and life is uncertain, this thought creates loads of anxiety in people with OCD. The anxiety in turn compels the person to engage in compulsive behaviors in a futile attempt to arrive at certainty. 

Many people will have passing doubts, or get “cold feet” when they decide to marry. However, a person with OCD will persist in seeking evidence that they are marrying the “right” person. They may do this by repeatedly asking family and friends as to whether they like and approve of the intended spouse. They will compare their relationship with others. They take online relationship surveys and read blog posts on finding the perfect mate. They assess their day-to-day interactions with their fiancée, often rewinding and replaying conversations, to determine if they are a good fit for each other. 

By the time patients with relationship OCD seek treatment they have often been engaged in compulsive checking, reassurance seeking, and rumination for many hours a day and for many months. And even though they are exhausted from this behavior, there is often reluctance on their part to see their fear of making this decision as just another form of OCD. They say, “But this is really important! I’m making a major life decision.” And I respond by saying, “Yes, but life is unpredictable. You have no idea how things will turn out. Do you want to carry on with constantly assessing whether this person is the perfect person for you, or do you want to get on with your life?” 

Since most of these patients have never sought treatment for OCD in the past, I explain how the content of their obsessions is irrelevant. I ask if there were other times in their lives when they had intrusive thoughts that demanded safety and certainty that bad things wouldn’t happen. And did they try to alleviate the anxiety caused by these thoughts by performing mental or behavioral rituals. Typically, I’ve found that they’ve struggled with OCD in the past, but had no label for it. Usually my patients come around to understand how OCD has impacted their lives in other ways outside of their relationship OCD.

When my patients understand how OCD demands certainty, regardless of the content of the intrusive thought, tackling their relationship OCD makes much more sense. I help them to stop checking with others about the suitability of their intended partner. I ask them to refrain from reading anything about choosing a spouse. I help them to limit ruminating (compulsive mental checking) about the pros and cons to marrying their fiancée. I ask them to embrace uncertainty and be willing to sit with the anxiety caused by the thought that they may not be marrying the “right” person. I emphasize that life is unpredictable and to live life fully one has to take risks.  Once my patients understand this and stop engaging in a futile quest for certainty, they are able to move on with their lives.

Before 5th of January 2018 i was living joyful happy life until i got a flu in the same month when i sat one of the beds of the clinic i felt like i am dying then i took 40MG Cortisone after that my routine changed for worse weeks ago i started again taking my seroxat 20mg everyday i wake up i say my health gone i wasn't like that years ago why this happened to after 33 years even my mom want me to get job and get marry i feel afraid how am i going to live with my wife i feel like i will ruin her life and her children i am one of the people can't stand kind toys i magine if i told her i have ocd she will hate me

Hi Hassan,

I really feel for you, I’ve gone through similar struggles. I was so scared my partner would not want to be with me if he found out about my OCD.
Eventually I got so bad I had no choice but to tell him. Thankfully, he has been very understanding and so supportive. After a lot of work I am slowly getting better now, and I’m sure you can too.

I just want to tell you that it can and does get better, and we often underestimate the people we love because we’re so scared of losing them. There are always risks in life - and that’s okay.

Was seeing someone with ocd but they called it off. Recently we have been in contact he said he missed me and wanted to give it a go once he got better. Was really positive but past few days he has been ignoring me says he doesn't want to and I should move on as he can't make anyone happy. Ocd ruined his marriage and he says it controls everything. Really like him.

I have been dating my boyfriend for 4 years and we have had a wonderful relationship - completely in love, committed, supportive of each other, have similar values, and come from similar socioeconomic upbringings. And we’re now 30. I have been wanting to get married but as it has become more of a reality, he seems debilitated by the idea and lays out all the reasons we may not be a good fit and that he doesn’t know for sure in his intuition. Yet when it’s a topic of something he wants to do together in the distant future, he talks about it with excitement. He knows he is the cause of the hold up and feels terribly. He has been seeking therapy for a while but it doesn’t seem to have changed anything. Does this mean it will never happen and I just need to face it and move on? It’s so painful and hard because it is truly inconsistent with the reality of our relationship and compatibility. Thanks for your help.

Hello my name is Joshua Duncan, I am 27 years old, and I have had OCD for last the 15 years. Within the last month I have found that I now suffer from relationship OCD as well. I have been with my girlfriend for almost 2 years now. However in the last month my dad cheated on my mother and tried to commit suicide. A week later my girlfriend and I had a miscommunication and i feared we were going in different paths and i freaked thinking i needed leave my girlfriend. I got myself back on meds but still will have intrusive thoughts about it. I love her dearly and want nothing but to be with her. What should I do?

Im going through the same thing my friend you're not alone and you're not going crazy buddy this is a real mental illness.
Im 26 years old and my fiances and I have been together for 1 year now and shes 7months pregnant...Im going to be a dad real soon trust me Im getting somewhere with this.
Ive been having a lot of unwanted thoughts similar to yours, a few weeks ago i had a mental breakdown with all the stress of being a new dad and my own. Bi polar mood swings, depression, anxiety and sympathy pregnacy issues made things much much worse. The thoughts consisted of: running away, my ability of being a good dad, my fiances and the one that bothers me the most was questioning my love for my fiances which is outrageous because I know I love this woman to death. The emotions I felt I was very angry, afraid and extremely sad all at the same time and I ruminated about it because I didn't understand why it was happening my subconscious mind did though and I hated it because they wont go away so i told my fiance and I thank God for blessing me with someone so supportive and understanding that made things so much better and it makes thing easier to deal with even with the random intrusive thoughts. I found away to deal with it now that im clear headed; your life is gonna have ups and downs even in relationships
(Stereotypical thing to say, I know) but its very true I promise.
Just remember when you have downs keep in mind that its going to go up from here. But dealing with the downs seems impossible but it can be done even with mental illnesses no one said it was gonna be easy my friend. What i do is meditate or keep my mind occupied and have a positive whatever you're not there attitude towards these thoughts when they flare up, remember the good and enjoy the time you have with your partner here's a video (whoever is also reading this might benefit from it too)
listen to it when you go to sleep at night or laying down to take a nap.
Remember its all in your head and its not what you truly want you don't want to leave and you know that remember to tell yourself that everyday and things will get easier. sometimes if you need to medication helps too. I hope this helps you man and anyone reading this.

Another question is it okay to turn your negative thoughts into positive ones so you can start creating a habit and believing good things happen
